• Seniors - Transcript Request Process

Directions: Follow the instructions below based on how and where you need your transcript sent. All transcript requests are processed in the order received within 5 school days.

    1. Electronic Delivery to Colleges Listed in Naviance (Colleges I’m Applying To)

      • Add college(s) in Naviance under “Colleges I’m Applying To”

      • Check Naviance: there must be a blue computer symbol showing under “Submission Type” in the “Colleges I’m Applying To” section or else we can’t send electronically through Naviance and you must request another option

      • Make sure deadline and delivery type is correct in Naviance

      • Colleges should be in Naviance at least 5 school days before a deadline to ensure the transcript is sent by the deadline, and you must have reviewed your Naviance college list with your counselor, Ms. Fitts, or Mrs. Preston in the College & Career Center before this time.

      • Complete the Initial Transcript Request Form

      • Note: If a counselor letter of recommendation is needed, a counselor meeting must be scheduled at least three weeks in advance of a deadline. If your colleges do not require a counselor letter of recommendation but they do require a transcript, schedule a meeting with Mrs. Preston.

      • Click here to schedule a meeting with your counselor or Mrs. Preston.

    1. Mail Delivery to a college or scholarship program

      • Complete the Initial Transcript Request Form

      • Must provide a pre-addressed and stamped envelope to Ms. Alba, OPHS Registrar, in the main office.

      • Make sure to attach your first and last name somewhere on the envelope (e.g. post-it, paperclip) that you provide Ms. Alba.

    1. Email or Upload to a college or program NOT in Naviance - Use this request FORM.

      • If you need an unofficial copy, you may  download one from Naviance under “Colleges > Manage Transcripts.”

    1. Mid-year Report: If one or more of your colleges requires a mid-year report and transcript, please complete this Google form by Friday, January 16, 2026. If you request a mid-year report after this date, you must email your counselor directly.

      • In order to determine whether your college(s) require a mid-year report, you can check the application requirements on each of your college’s admission websites. For Common Application colleges, you can check by looking at the “My Colleges” page on your Common App account, and then viewing the “recommendation requirements.” Click here to view an example.

      • UC, CSU, and community colleges do not require a mid year report unless a campus contacts you specifically to request one.

      • Counselors will send mid-year reports in late January 2026. Please contact Ms. Fitts or Mrs. Preston with any questions.
