If your child has a food allergy, please submit a Medical Statement to Request Special Meals and/or Accommodations form to firstname.lastname@example.org. The form must be completed and signed by a licensed physician. Upon receipt of the completed form, we will reach out to work with you to determine the most appropriate menu options to meet your child's specific needs.
For the safety of your child, we kindly ask that you refrain from sending your child to get meals from the cafeteria until this process is complete. Our standard menu offerings are always subject to change without notice and may contain ingredients that may not be suitable for your child.
