• We are currently only enrolling students who are residents of Oak Park (zip code 91377)

    Below are the procedures and required documents for enrollment:

    • Complete, submit and print the district pre-enrollment document - CLICK HERE
    • Proof of residency (Escrow documents or Lease/Rental Agreement and 2 utility bills)
    • Up to date Immunization record. Please note the required T-dap vaccination for 7th and 8th graders
    • Copy of original birth certficate or passport
    • A current report card or exit papers from your student's school
    • A Completed Request for Records form.
    • Completed Home Language Survey

    NOTE: All enrollments will be done completely online.

    ***If you are currently enrolled in any 5th grade class within OPUSD you DO NOT need to enroll at MCMS for 6th grade.
