• Registration Requirements


    Please contact the Brookside office at (818) 597-4200 for registration information.


    The following items are required to complete your child’s registration.

    • Proof of  Residency
          property tax bill or rental agreement or escrow paper
      one utility bill, telephone bill will not be accepted.
          California driver’s license or car insurance with current address on it

    • Immunization Record – We need to see a copy stamped by the doctor’s office.  Please bring a copy that we may keep on file.

    • Original or Certified Copy of Birth Certificate  -- For Incoming Kindergartners.  We must see the original or a certified copy.  But, please bring an additional copy that we may keep for our files.

    • Complete address of  former school

    • For Kindergarteners: Bring your personal calendar to facilitate scheduling of  assessment testing.

    The above listed items are all necessary to fully complete your child’s registration and your child will not be admitted to school in August unless all items have been submitted.
