Dates and Times


12th Grade Registration Tuesday, Aug. 21: 8:30 am - 12:00 pm

11th Grade Registration Wednesday, Aug. 22: 8:30 am - 12:00 pm

10th Grade Registration Thursday, Aug. 23: 8:30 am - 12:00 pm

9th Grade Registration Friday, Aug. 24:  8:30 am - 12:00 pm  
Please allow a minimum of 1-hour to complete registration. Doors close promptly at stated time.
Do not arrive at the last minute. There will be no make-up day.

Please bring all completed forms!

 
By law, students who do not have proof of receiving a Tdap booster shot will not be allowed to attend classes until proof is provided to the school. The tetanus-diphtheria booster shot (Td) will not meet the requirement. There are no extensions or grace periods allowed.

Forms to Print and Return
Contains forms that are to be completed and returned to school. All forms are in PDF format and you may type information onto each form using Adobe Reader. To help our school office easily read critical contact information please TYPE the information on Form 1. Thank you for your help. You may save your typed data; simply click on "Save As" to save the form to your desktop. Be sure to use Adobe Reader to type your input and not your computer's "preview" application.

    What is Registration?
    Registration is the term used to describe all activities leading up to and associated with the start of school. At OPHS, Registration is typically held over a 4 day period in August (prior to the first day of school) when all completed and signed forms are turned in, school ID photos are taken, student class schedules are distributed, and other student and parent activities take place which are necessary to ensure a smooth start to the school year. All students, including newly enrolled students, students on permits, and all returning students, are expected to participate in Registration activities.
